 Homeowner Assistance Fund (HOAF)

​​​Required Documentation​

  1. State-issued photo ID (must not be expired)
  2. Written statement from homeowner detailing COVID hardship
  3. Documentation of current income
    • Current paystub(s)
    • YTD profit & loss statement if self-employed
    • Current year SSI/SSDI statement
    • Documentation of other income you are currently receiving such as short-/long-term disability or child support
  4. Proof of ownership for subject property (deed)
  5. Property valuation (PVA statement or current tax bill reflecting property value)
  6. Most recent bank account(s), all pages
    • Deposit accounts (checking, savings, certificates of deposit, etc.​
  7. Most recent statement(s) for assistance requested
    • Mortgage statement
    • Past due property tax bill, can be no later than 2017 tax year
    • Past due Homeowners or Flood Insurance bills, if not part of mortgage escrow
    • Past due Homeowners Association Dues bill
    • Past due utility bills (water/sewer, electric, and gas)
    • Executed partial claim​​
The Housing Counselor may ask for additional documentation not included on this list, if needed.
